PAD Cover Pages
If you desire to add cover pages to your PAD to organize documentation, you may use the templates cover pages in the table provided below. These templates can be customized by you and added at the beginning of each PAD section. The templates provided below are not inclusive of all of the activities and documents you may need to include in your PAD. The images below display an example of where in the PAD you would upload these cover pages, and how they would appear to the reviewer.
